The participant’s spouse or covered dependent children may continue coverage up to 36 months following the event of divorce (or legal separation) if the Request for Continuation of Eligibility form is submitted to GuideStone within 60 days after the plan coverage would otherwise end. You must pay all monthly charges to GuideStone. You must pay these costs when due, or your continued coverage will end.
