Once a member accesses Join.SwordHealth.com/BCBS, a member will complete an initial screening questionnaire. If determined to be a candidate for the program, the member is enrolled and presented with the next steps.
- The member schedules a video call with the Sword physical therapist (generally within 24-48 hours, depending on the chosen appointment time by the member). During the call, the physical therapist assesses the member’s condition, goals, etc., to ensure they are a good candidate for the program.
- The physical therapist develops a member-specific exercise plan.
- The member is sent their digital therapist kit (a tablet with a customized exercise program and wearable motion sensors that guide the member to perform the exercises correctly; track program engagement/activity).
- The member follows exercise sessions using the tablet and the sensors and connects with their physical therapist at a minimum of two times per week
- The physical therapist uses the member data captured in the portal and periodic member surveys to manage and drive the member’s program plan.
- Throughout the process, the member receives ongoing support from a physical therapist via the member’s preferred communication channel (e.g., chat, phone, email, video).
- The SWORD Health mobile app provides educational articles and cognitive behavioral tools for ongoing support.
